Please prepare the documents applicable to your category. All submissions must be complete and in high-quality format to ensure smooth processing.

📌 General Requirements (All Applicants):

• Old and New Passports

• Two (2) recent photos, US visa size (2x2 inches, white background)

• Original Bank Certificate

• Original Bank Statement (last 3 months)

• Original Copy of Land Title (if applicable)

• Original Copy of Car Certificate of Registration (if applicable)

• Original PSA Birth Certificate

• Original PSA Marriage Certificate (if married)

🏢 For Self-Employed Applicants:

• DTI / BIR Business Registration

• Business Permit / License

• Latest Income Tax Return (ITR) – Form 1700

👔 For Employed Applicants:

• Certificate of Employment

• BIR Form 2316 (latest)

• Approved Leave of Absence from Employer

🤝 If Sponsored by a Company:

From Sponsor Company:

• Affidavit of Support

• Business Documents (DTI or SEC Registration, Business Permit)

• Bank Certificate

• Latest Income Tax Return

From Applicant:

💻 Documents Needed for Online Application:

• Copy of Passport (scanned)

• Soft copy of 2x2 photo (white background)

• Draft of DS-160 Application

• Fee: PHP 14,000
  (Inclusive of U.S. visa fee and documentation support)

📦 For Dropbox (Visa Renewal via Interview Waiver):

• Original Old Passport with Previous U.S. Visa and New Passport

• Soft copy of 2x2 photo (white background)

• Hard copy of 2x2 photo (white background)

• Draft of DS-160 Application
