🔍 GENERAL GUIDELINES

• Personal appearance required for biometrics (by appointment)

• Submit at least 1½ months before intended travel date

• No passport borrowing allowed

• No travel during visa processing period

• Processing time: Minimum of 15 working days from biometrics/filing

• Embassy may request additional documents as needed

📄 Basic Requirements

• Duly filled-out Temporary Resident Visa Form (IMM5257)

• Duly filled-out Family Information Form (IMM5465)

• 1 recent passport-size photo (white background)

• Original valid passport (valid at least 6 months from departure)

• PSA Birth Certificate

• If traveling with spouse: PSA Marriage Certificate

👤 If Employed

• Original Certificate of Employment

• Photocopy of company ID (bring original during appointment)

• Latest ITR (photocopy)

👔 If Self-Employed / Business Owner

• Photocopy of:

  • DTI / SEC Registration

  • Mayor’s / Business Permit

• Latest ITR (photocopy)

🩺 If a Doctor or Professional

• Hospital certificate

• Photocopy of PRC ID

• Bring original PRC ID during appearance

💰 Financial Proof

• Original bank certificate

• Original bank statements (latest 6 months)

✈️ Travel Itinerary

• Roundtrip flight booking certificate with detailed itinerary

• If attending:

  • Conference/Meeting/Convention: Event registration + daily schedule

  • If invited: Official invitation letter

🤝 If Sponsored

• Sponsorship letter from sponsoring company

• Photocopy of:

  • Business permit

  • Latest ITR

• Sponsor’s original bank certificate & 6-month bank statements

👶 For Children / Minors

• Parent/guardian must submit full documents as listed above

• Affidavit of Support

• If studying:

  • Enrollment certificate from school

  • Photocopy of school ID

• PSA Birth Certificate of child

• If traveling alone or with a guardian:

  • DSWD Travel Clearance
