1. Dully filled up visa application forms(any size)
2. Photocopy of valid passport data pages with 6 months “validity from the date of travel and copy of all valid and previous visa stamp/s
3. Two (2) pcs. Recent passport size pictures (colored white background)
4. Employment Certificate (if employed)
5. Certificate from hospital (if physician)
6. Business Registration, Copy DTI and SEC (If Self – employed)
7. Recent Bank Certificate
8. Bank Statement for the last 3 months same bank account
9. Annual Income Tax Return
10. Marriage Contract (If Accompany Husband or Wife)
11. Birth Certificate (if Accompany Child)
12. School certificate (if student)

Additional requirements:

1.Notarized affidavit of support(if supported/sponsored by another person/parents

2.Proof of financial documents from the accompanying person/sponsor
